Re-Enrollment Process When Re-Enrollment time approaches you will receive en email with a link and instructions to complete the online Re-Enrollment Packet. The link will take you to the FACTS Management website and you will find the Student Enrollment Packet under Family in the Facts Family Portal. 1. Students must re-enroll to guarantee their space in school. 2. Complete and submit the online Re-Enrollment Packet with the non-refundable fee. The fee must be paid for the packet to be completed. 3. Please bring, send, or attach (in online packet) the following documents: o Current original Immunization Record and Physical provided by Florida doctors/Health Department. Upon completion of the online Re-Enrollment Process; registration will be finalized at the school. Finances Once the online Re-Enrollment Process is completed, please contact our Business Office at 305-220-5955 to schedule an appointment to sign the Financial Agreement. Class Schedule (6 th to 12 th grade students only) Once the online Re- Enrollment Process is completed, please contact our Office at 305-220-5955, to schedule an appointment to prepare the Class Schedule. Uniforms and Lockers School uniforms are sold in the office. Please see our Handbook for uniform requirements. Lockers are assigned by the Receptionist at the school office. Lockers are only for 6 th to 12 th grade students.
